Upload your Picture for player card (head-shot) and copy of Photo ID

Step 3

Make Payment and Submit

Replacement $15.00 

Step 4

Your temporary pass will be emailed to you immediately (yes, you can play tonight) and your physical card will be mailed to you the next business day.
