Injury Claim Process
GSSL/USCS player cards purchased on/after April 29, 2019 do not include secondary player accident insurance coverage.
If you are playing with an unexpired Cascadia/GSSL membership card and are injured during a GSSL scheduled/sanctioned game or sanctioned tournament, you are covered by a secondary player injury medical insurance policy that will cover up to $5,000 of your recovery costs.
To file a claim, contact the Cascadia Adult Soccer office by emailing the office using cascadiasoccerleague@gmail.com. The office will email you an Injury Claim Form Packet that includes claim forms and instructions. It is important that you contact the office as soon as possible as the
forms must be returned to the Cascadia office and submitted to US Club
Soccer (by Cascadia) within 90 days of the date of injury.
You must complete and sign the first page (Part A) of the USASA Special Risk Accident Claim Form. The second page (Part B) will be filled out by the Cascadia office. There is also a Claim Verification Form that must be completed and signed by someone who was present at the time of your injury (preferably a team manager or coach).
Return the completed and signed forms to the Cascadia office. The office will confirm that your player card has not expired and that your information in the Cascadia player database is up to date. Once the Cascadia office has completed the verification process, all documents will be sent to US Club Soccer who will verify your information and status, sign off on the forms and forward them to USASA and the insurance company. The insurance company will send you a claim acknowledgement letter that explains the reimbursement procedures.
